Call Nomi Health at 855-601-1900 before you schedule any surgery to go through their providers.

*Members on the Basic Plan (HDHP) (HSA) must meet an annual $1,600 individual/$3,000 family deductible paid to Nomi Health after which the Plan pays 100% of all services if you use a Nomi Health provider. If you visit another provider that is not part of the Nomi Health program, you will be required to meet the $6,000 deductible.
How does Nomi Health work?
Physician Recommends A Specific Service
In order to be a candidate for Nomi Health services, you must have valid medical records and a referral order for the specific service needed (physical therapy, surgery, etc.).
Chat With A Care Guide
As soon as you know the medical service needed, call or email Nomi Health to speak to one of our Care Guides. Your Care Guide will find the nearest physician/facility for your specific need and determine any out-of-pocket costs.*
Schedule With Physician/Facility (you MUST use a Nomi Health physician/facility in order to benefit from this program)
Once we locate a Nomi Health physician/facility for the service needed, the physician/facility will reach out to you to schedule your appointment. If pre-authorization is required, a Care Guide will work to get all necessary approvals.
Receive A Voucher
Before your appointment, a Care Guide will provide you with a voucher to present to the physician/facility on the day of your visit. This voucher will replace the need to show your major medical ID card.
